Delivering care to the second most populated county in the U.S., Cook County Health & Hospitals System (CCHHS) of Chicago serves a large and diverse patient population. Many of these patients suffer from chronic diseases such as lupus, HIV, and cancer all of which require consistent laboratory monitoring. CCHHS found that many patients with chronic conditions struggled to keep up with their bloodwork and frequently missed appointments.
